Date Last ModifiedQC Forms Management allows for creation and organization of all Quality Control forms.
Options:
This button creates a new evaluation form.
This button edits the properties of the currently selected form
This button creates a DRAFT duplicate copy of the selected form
This button deletes the form. Please Note: This is different than making a form Obsolete. Deleting a form removes the form as well as all evaluations and data associated with the form. If you need to retain evaluations and data, choose to make a form Obsolete.
This button launches the form in a test view, to see how the form will appear to managers on the Front end.
Note: You can only edit or add sections and criteria to forms that are in DRAFT mode. Finalized forms only allow you to edit the section and criteria name (not the value) as well as change the order of the sections and criteria.
Form Types
This is a form that does not require a playlist to be attached to it. Usually for a general evaluation form.
This form requires a playlist. One or multiple call recordings must be attached to this form to be evaluated.
Form Status
Green means the form is active.
Brownish Orange represents forms that are in draft mode and can be edited.
Gray are forms that are now obsolete and no longer visible on the front end. However, reports and data will still be available for the form.
Forms are broken into two parts.
Each Section contains criteria. Sections can be worth different percentage value. When completed, all sections must add up to 100%.
Within each section, Criteria may also have different weights. The more points awarded to a specific criteria, the greater weight it is used in calculating the SECTIONS overall percentage.
Training Material
Training Material may be assigned to each Section and to Each Criteria. When sending a completed QC form out for review, the option to include Training material will appear. Training material may be an internal or external link to your preferred training program.
Preview
The Preview section shows how your Evaluation Form will appear on the front end.
Templates
You may save criteria that you use often as a template. After creating the basic structure of the criteria, select the checkbox near the bottom of the Criteria Form. Here you may name your template and select it from the drop down (Top of Criteria Form) in the future.
